Registration

1. Please read the information sheet carefully.

2. Fill in the FSP registration form now.

3. Ensure that all the necessary documents (page 3 of the registration form) are available (photocopied on one side if possible, not stapled). Then choose an affiliated association and complete the affiliated association's registration form.

4. Send the completed SPF registration form and enclosures to the desired affiliated association, together with the affiliated association's registration form. The affiliated association will then forward the documents to the SPF.
